Physical Therapist
SALMON VNA & Hospice seeks Physical Therapist to join our growing healthcare team.
Full Time, Part Time and Per Diem Shifts; Flexible weekday and weekend rotations.

Our VNA and Hospice facility in Milford offers customers exceptional home health and hospice services provided by dedicated, professionally licensed clinicians. Our interdisciplinary team is devoted to optimizing quality of life, resulting in greater peace of mind for you and those you love.
Responsibilities
o Conduct initial and on-going assessment of the patient's physical, psychological and social needs
o Provide skilled physical therapy to restore and maintain patient's optimal mental/physical health
o Create individualized patient plans utilizing professional knowledge and judgment; fulfill through teaching, counseling, and rehabilitative measures with residents
o Establish plan of care for HHA and PTA, providing supervision and required updates
o Maintain OASIS data and patient documentation in timely, accurate manner
Requirements
o BS Degree and Graduate of an accredited School of Physical Therapy
o Current Massachusetts PT License and CPR Certification
o Valid Driver's License
o Minimum 1 year of PT experience; 2 years Community Health experience preferred
o Strong written and verbal communication skills, with patients, families, and health care personnel
o Great time management skills
o Ability to work effectively as a part of a team, with ability to function independently
