Clinician - Adult Community Clinical Services

We are seeking a Primary Clinician for our Adult Community Clinical Services (ACCS) program.

The Primary Clinician provides care coordination through development of a collaborative relationship with the person engaged in services, and connections with family, other providers and community supports that are important to the person. They will be experienced in the use of interventions to provide person-centered assessments and service planning, and advocacy, coordination of care, and linkage with other services and the community.

As the Primary Clinician the candidate will emphasize skill building that focuses on sustained engagement, independent housing, community tenure that is uninterrupted by hospitalizations, symptoms stabilization, self-management, and increasing natural supports that ultimately leads to successful transition to more independent living situations.?

Schedule: Monday-Friday (one weekend day is encouraged)

Required Skills

  • Must have excellent written and verbal communication skills (in the English language) and the ability to make effective public presentations.

  • Must be able to work some evening and weekend hours, provide 24-hour on-call coverage on rotation, and to respond to emergency calls as necessary.

  • Must have valid driver?s license and available, dependable transportation and be willing to provide transportation to person's served in personal vehicle, as needed.

Required Experience

  • Master?s degree and licensure in a related clinical discipline required

  • At least 1 year related experience required

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
